Personalised Care presents a new relationship between people, professionals and the health care system.

It improves people's health and well-being, joins up care in local communities, reduces pressure on NHS services and helps the health and care system to be more efficient. It also ensures patients receive care that is tailored to their needs which can have a significant impact on their experience and quality of life. It is achieved through a series of supportive conversations in which the patient, or someone who knows them well, actively participates to explore the management of their health and well-being. 

There are four interventions across all pathways. 

  • Holistic Needs Assessment (HNA) 
  • Personalised Care & Support Plan (PSCP)
  • Cancer Care Reviews (CCR)
  • End of Treatment Summaries (EoTs) 

A simple questionnaire for patients, helping you to identify any patient concerns, start a conversation about needs, develop a Personalised Care and Support Plan, share the right information, at the right times and signpost to relevant services. The assessment can be carried out at any stage of the cancer pathway.

Ensures people's physical, practical, emotional and social needs are identified and addressed at the earliest opportunity. It allows you to keep a record of all conversations, decisions and agreed outcomes, understand a patient's care and support needs, their life and family situation and know what is required to make the plan achievable and effective.

This is a conversation between a patient and their GP or Practice Nurse about their cancer journey. It helps patients talk about their experiences and concerns, understand what support is available and receive the information they need to begin supported self-management. 

The Quality and Outcomes Framework (QOF) requires cancer care reviews to be carried out at the time of a patient diagnosis (within 3 months) and after a patient has received acute treatment (within 12 months).

This is a document produced by the hospital clinician at the end of the initial treatment for cancer, which is to be shared with the patient and their GP.  This can include an overview of any treatment the patient has received, details and any potential side effects of treatment, signs and symptoms of cancer recurrence and any contact details to address any concerns.
